What is the Chronic Disease Management Plan Program?
The Chronic Disease Management (CDM) Plan is an initiative by Medicare that allows eligible people with a chronic condition (6 months or longer) to access up to 5 allied health services to help with the management or treatment of their condition.
What sessions can I use my plan with?
30-minute or 60-minute private sessions with an Exercise Physiologist are eligible under the plan.
Who can my sessions be with at Wollongong Pilates Studio?
Sessions must be with an Accredited Exercise Physiologist. Our Exercise Physiologists are Donna Oliver and Shauna Downes.
It is likely your GP has recommended you work with either Donna or Shauna on your Care Plan referral.
How much are sessions?
60-minute sessions - $135 (out of pocket cost $76.70)
30-minute sessions - $83 (out of pocket cost $24.70)
How much will Medicare rebate me?
Medicare will rebate $58.30
After paying your account in full, the Medicare rebate can be processed by our team and funds will be available in your account within a few days.
How long do I have to use my care plan?
You may be eligible for up to 5 sessions per calendar year. If you do not use all your sessions in the current calendar year, they may carry to the next, however will contribute to the maximum (5) number of sessions in the next year.
How do I book in for a session?
As sessions are private, they won’t show online. To book in for a session, please email wollongongpilatesstudio@gmail.com with your preferred dates & session times.
30-minute session not long enough?
If our studio is attended with staff, you may continue undertaking the exercises prescribed in your session by your Exercise Physiologist in the studio for another 30 minutes.
